Languages

The Consent Form comes in two versions: long and short. The long is in English with Spanish on the back, English with Cape Verdean Creole on the back. The short paragraph is in English, Spanish and Mandarin.

 

Passive consent

It's a "passive" or opt-out consent form because it's returned only to withdraw permission to participate.

Can schools set their own deadline for parents to return it?

Yes, and... There is a sentence in the form that can’t be removed ("If you do not return this form before the first day of the course, we will assume that we have permission for your child to complete the survey…") so have a backup plan if a form arrives late.

Outward Bound is partnering with The PEAR Institute (Partnerships in Education and Resilience) on a research project to promote our students’ positive social-emotional development. As part of this effort, Outward Bound staff may invite your child to complete a brief survey that is given to students on Outward Bound courses. The survey includes PEAR’s Holistic Student Assessment (HSA) plus several additional questions about students’ social-emotional development and experience while on the course. Your child’s responses will be kept confidential, results will be reported as a group, and his/her name will never be used in any reports related to this research. Data from this survey will be used for research and educational work and only designated Outward Bound staff and Outward Bound’s research partners will have access to the results.

Which one do I use?

For 2020, either will work. The paragraph is all that is required, but the longer page was given to schools earlier in the year so that they could plan ahead and get forms to families.​

How does it get delivered to parents?

Schools decide how to deliver this to parents. Options include sending it with other admissions paperwork, sending it as part of what your program already does, or emailing it to parents directly.  For group programs, deliver the form to the group's contact person, ask them to get it to parents, and leave it at that.

The short paragraph can be added to another form if that is easier; lack of consent just needs to be noted on the Crew Roster if it is returned.

Do students 18 and up need a form?

No consent form is needed for ages 18 and up. Voluntarily completing the survey implies consent.

What do schools do with them when they are received? Do we send them to the Lab?

Use them to complete the "Has parent consent" section in your crew roster. A returned form withdraws consent, so enter "no" for that student. Then ensure that student doesn't participate. After that, they're no longer needed. You don't need to upload or mail them to the Lab, because the roster tracks that.
