Paper Survey Introduction Script

Hand out the surveys then read this to students

The survey you’re about to fill out helps us improve our programs. It asks about your experience, thoughts and feelings throughout Outward Bound. It’s not a test. There are no right or wrong answers. Please don’t skip any items and don’t talk to anyone else when you fill it out. If you don’t understand something, please ask me. Your answers on each item are private; no one else will see these. If at any point you feel you can’t continue, please let me know and we’ll talk about it.

This survey asks you to think about how you felt at the start of your Outward Bound experience and how you feel right now. Let’s practice one together. Think back to the start of the course. What time and day did the course start? [Give time to think about this.] What time and day it is now? [Help students compare the answers.] Let’s practice another one. Name the first person you met at Outward Bound. Name the people you now know at Outward Bound. Let’s do one more. What was the first activity the group did together? What activities did you do today?

Now, let’s look at the survey. Let’s practice the first one together. It’s called “Practice.” It says, “I like to be outside.” Think about how you felt at the start of Outward Bound. Now fill in one bubble that matches how you felt at the start. Next, think about how you feel now, at the end of Outward Bound. Fill in one bubble that matches how you feel now. For each question, fill in both responses before moving to the next one. Does anyone have questions? Please take your time and answer honestly. When you’re finished, quietly bring your survey to me. You may begin.

Electronic Guide, Roster Cover Sheet and Surveys

There is only one link for all electronic Rosters, regardless of school.

In order to use electronic Surveys, you MUST use the electronic Roster. Paper Rosters will not work any longer for electronic Surveys. 

​

By clicking on the link below or using the QR code, you will get access to an electronic version of the Roster. Fill out the Roster on any device, and you will be assigned a Crew ID, which you will receive in an email. Then you will be given a link and a QR code to access the surveys. Students can use computers, tablets, or phones. 

​

Double-check?

For surveys, ensure everything is filled out, or if something's missing it's deliberate and not an accident. It only takes a handful of seconds per survey and saves a ton of time later. For rosters, everything needs to be filled out. Remember, complete and accurate data at every step.

Who do I give the completed roster and surveys to?

This depends on your school's systems. Most likely you'll do one of these: 1) hand them in with your paperwork; 2) give them separately to your course director; or 3) turn them in directly in to someone else (e.g, your location's survey manager or point person). From there someone will get them scanned, uploaded, and mailed.

Is it okay to use pens, pencils, markers to complete the survey?

Pencil is best in the event students want to change any answers, but it must be dark. Light pencil won't be readable when scanned. Black or blue ink will also work. Don't use markers as they bleed through to the backside and look like responses on the opposite page when scanned. Don't use crayon.

What if a student has permission but doesn't want to participate?

We want students to participate but ultimately it's voluntary and we can't force them. Encourage their participation and explain the benefits (e.g., we value their feedback, it gives them a voice, their responses help improve programs). Ask what their concerns are and help them feel more comfortable.

If the student still refuses, then they do. Do not fill out their Survey ID on the roster.
